Observation

Licensee Tamaya Edwards is not complying with the Governor’s mandate regarding vaccines. She has stated that she is not vaccinated and she had no other document to meet the COVID 19 vaccine requirement about which she acknowledged her awareness that it was a requirement since the middle of October. She mentioned intending to get vaccinated against COVID 19 but had not "yet" done so. Note: there will be an additional SAFETY PLAN.
